Please look over the directions, complete the form with the correct information, and return it to the Consular Office with the required attachments.
Please note that our Consular Office is in a separate location from the Embassy.

General Requirements
1. Click below to download the Power of Attorney Document.
2. Please note that it does require the Adobe Acrobat Reader
3. Once downloaded, select File > Print.
4. Choose your Printer under "Printer Name" then click Properties.
5. Click "Advanced" and choose paper size "LEGAL".
6. The Power of Attorney Document is 8.5 x 14 so be sure to use "legal-sized" paper.
