General Requirements
1. Click below to download the Power of Attorney Document.
2. Please note that it does require the Adobe Acrobat Reader
3. Once downloaded, select File > Print.
4. Choose your Printer under "Printer Name" then click Properties.
5. Click "Advanced" and choose paper size "LEGAL".
6. The Power of Attorney Document is 8.5 x 14 so be sure to use "legal-sized" paper.
