Please look over the directions, complete the form with the correct information, and return it to the Consular Office with the required attachments.

Requirements for an Entry Permit:

  1. One Entry Permit form.
  2. If you are a U.S. alien resident, provide a copy of your Green Card (front and back), your Social Security card, or evidence to prove your Afghan citizenship, along with one type of photo identification.
  3. Please notarize your Entry Permit application. If you are applying in person, notarization may not be required.
  4. Passport copies of both parents are required for minors

Afghans born in Afghanistan, holding passports other than an Afghan passport, need not apply for a visa. An entry permit may be issued from the consulate, if requested.
